Current GRCCO club members will have first access to making appointments until 2/22/2018 and then times will be assigned on a first reserved, first selection basis.
You will need to bring your dog’s date of birth, AKC/CKC registered name and number, ID number (tattoo/microchip) and the sire and dam’s registration number. Filling out the OFA forms online before you print them makes them very easy to read for future reference. Please tell me how many dog(s) you want scheduled for which exam(s) and your appointment time preference and I will do my best to accommodate you. Forms take about 5 minutes EACH to complete.
** The forms have the examining veterinarians note whether they did or did not verify the tattoo/microchip on this dog. We will have a scanner available so this verification can be noted on the form.
Pay the day of the Clinic. We accept only checks payable to GRCCO or an EXACT CASH amount. Please be responsible. GRCCO is able to keep everyone's expenses to a minimum as long as all of the checks clear. ;-)

EYE CLINIC: $31.00 per dog Ophthalmologist Dr. Anne Gemensky Metzler, D.V.M., MS, D.A.C.V.O.
Have your dogs examined for ocular disorders including cataracts, progressive retinal atrophy, entropion lids and distichiasis. Dogs used for breeding should definitely be tested for genetic eye disorders. All owners should want to know what is happening with their dog's eyes and how well they can see. Registration applications will be available. See below for information needed to complete the form which is valid for 1 year from exam date. Minimum recommended age is 8 weeks, recommend checking by 6 months of age. (If a form is messed up, $2.00 fee is added for a 2nd one.)
***As of November 1, 2012 the ACVO will be endorsing the OFA Eye Certification Registry as its recommended canine eye registry. Diplomates of the College may submit eye registrations for either the OFA ECR or the CERF programs. More information is presented below and also will be updated off of the www.ofa.org website.***
EXAMS - There will be no changes to the existing exam protocols. The standard exam process will still be conducted by qualified ACVO Diplomates assessing dogs for the presence or absence of observable hereditary eye disease.
RESULTS - There will be no changes to the interpretation or classification of the exam results. Dogs with normal eye exams will receive OFA eye clearance numbers similar to the OFA numbers issued for hips, elbows, cardiac, etc. Dogs with observable, but passing conditions (currently known as Breeder Option Codes), will be issued notated OFA eye clearance numbers. Dogs with observed eye diseases of significance will be reported as ineligible for eye clearance numbers. A complete list of Breeder Option Codes and Ineligible Categories will be available on the OFA website soon.
VALID PERIOD - OFA eye certification numbers will be considered valid for one year from the time of the exam.
FEES - Fees for both initial submissions ($12) and resubmits ($8) of passing results for eye certifications will remain the same with one change. In order to maximize the benefits to breeders it is important to encourage owners to share all exam data, both normal and non-passing results. To that end, there is no charge to owners to submit non-passing results to the database if they authorize open disclosure.
ONLINE ACCESS - All normal/passing eye exam results submitted to the OFA will be available on the OFA’s website. Non-passing results will be available on the OFA website if the owner authorizes disclosure.
AKC - OFA eye certification numbers will be shared with the AKC for inclusion on their registration and pedigree documents if the dog was permanently identified via microchip or tattoo AND the ID was verified by the ophthalmologist at the time of the exam.

CARDIOLOGY CLINIC: $21.00 per dog Cardiologist Robert L. Hamlin, D.V.M., Ph.D., A.C.V.I.M. (Cardiology & Internal Med.)
Have your dogs examined for heart conditions including murmurs/sub-aortic stenosis. Dogs used for breeding should definitely be tested for genetic heart conditions. All owners may want to know the condition of their dog's heart. Minimum recommended age is 16 weeks with recommended pre-breeding screening after 1 year of age. DENTITION CLINIC: $16.00 per dog A veterinarian from Beechwold Veterinary Hospital will perform the examination.
The OFA has developed a new dentition database which is used to certify full dentition and is open to all breeds. There is no minimum age, however, prior to certification, all adult teeth must be fully erupted. The database is intended to certify that all adult teeth are fully erupted and present. It is not intended to certify compliance with any specific breed’s standard.

HEARTWORM/LYME/EHLERCHIA TEST: $27.00 per dog Beechwold Veterinary Hospital will provide results.
Heartworm is a debilitating and devastating disease that even with successful treatment can leave permanent damage. Make sure that your dog is clear before starting the preventative. The antigen test will be performed and indicates the presence of heartworms in any of the 5 cycles of the adult worms. A free retest is offered for any positive results.
MICROCHIP IMPLANT: $36.00 per dog Beechwold Veterinary Hospital can insert the microchip.
Have the AKC Companion Animal Recovery (CAR) microchip implanted underneath your pet's skin for permanent identification. The microchip is programmed with a unique, unalterable code that can be read using a hand-held scanner. This assists in the recovery of your animal. PATELLA CLINIC: $16.00 per dog A veterinarian from Beechwold Veterinary Hospital will perform the examination.
The patella, or kneecap, is part of the stifle joint (knee). In patellar luxation, the kneecap luxates, or pops out of place, either in a medial or lateral position. The form is available at www.ofa.org or http://www.ofa.org/pdf/plapp_bw.pdf ; (there is an underscore/underline between plapp_bw).
THYROID CLINIC: $90.00 per dog Beechwold Veterinary Hospital will send the sample to Michigan State University.
Hypothyroid (the gland does not produce enough of the hormone) is a concern of the Golden Retriever population. Symptoms may include obesity, skin or coat problems, infertility or sluggish behavior. Dogs used for breeding should be tested. All owners may want to test also. The thyroid maintenance medication is inexpensive and very effective. Tests to be performed include: Total Thyroxine (TT4), Total Triiodothyronine (TT3), Free T4 (FT4), Free T3 (FT3), T4 Autoantibody, T3 Autoantibody, Canine Thyroid Stimulating Hormone (cTSH), Thyroglobulin Autoantibody (TgAA) and Free T4 by Dialysis (FT4D). An OFA form with a fee of $15.00 will available for those who want to register their results. *For more accurate results, the dog should fast the night before the blood draw for the thyroid test.

***Handling Fee of $15.00 each is included as it must be frozen and sent on ice overnight.
*** Any dog having blood drawn needs to have fasted for at least 12 hours. ***
